The partners at Advanced Pediatrics have taken special care to develop a model for pediatric care which combines the benefits and services of a larger group practice with the personalized care of a smaller physician office. With over 35 years of experience providing quality pediatric care in our community, we have offices in Aurora, Centennial and Parker, and are welcoming new patients to all office locations! With our electronic medical records system, access to patient medical records is available from all three office locations - giving patients maximum flexibility in scheduling an appointment which best meets their needs.

Important Forms to Be Completed
In order to provide you with the best health care possible, it is important that our office receive a full copy of your child's health records, including immunization records, and that you complete our new patient forms prior to your first visit.

You may download and print a copy of our Medical Records Transfer Form by clicking on the link below:
Please give this form to your child's previous pediatric practice, and have them forward a copy of your child's records to our central medical records department at the address below:

New Patient Forms
We ask that all new patients complete the New Patient Information Form and the Patient History Form found in the links below. The New Patient Information Form must be completed online at least 2 days - 48 hours - prior to your first visit. (You will be prompted to first create an online forms account, which will also allow you access to other online services -- including requests for immunization records and prescription refills.) The Patient History Form may be downloaded, printed, completed and brought to your first appointment.
